Presumptive Disability

If you have patients in your clinic, that are terminally ill, and not already on permanent disability and medi-cal, if you fill out a form for presumptive disability (on iSite), with a short letter explaining the diagnosis, contact Connie Nguy to facilitate the patient's disability application -- which vastly speeds up the process. Connie will work with the Disability and Medi-Cal offices and it can make a big difference for our patients, shortening the process by many many months. It also helps our system because we get Medi-Cal reimbursement sooner. Patients who have had a CVA, and not improved over 3 months, also are eligible. This is extremely useful for patient newly diagnosed with cancer or recently severely
decompensated liver disease.
